DASD Facilities Request Form
ANY FACILITIY REQUEST FOR ATHLETIC FACILTIIES MUST BE FILLED OUT AND BE ACCOMPANIED BY PROOF OF INSURANCE, THEN TURNED INTO THE ATHLETIC DEPARTMENT.
IN ORDER TO GET IN AN APPROPRIATE TIME FRAME, PLEASE COMPLETE AND HAVE REQUEST TURNED IN TO BE APPROVED AT THE BOARD MEETING THAT PRECEDES YOUR FIRST REQUEST DATE BY AT LEAST ONE MONTH.
EXAMPLE:
IF YOU NEED A FACILITY REQUEST WITH A START DATE OF 5/01…SUBMIT PRIOR TO THE APRIL BOARD MEETING ITEMS DEADLINE (FIRST WEEK OF APRIL). THE BOARD MEETING FOR ANY MONTH TYPICALLY TAKES PLACE THE LAST THURSDAY OF EACH MONTH. THE SOONER YOU SUBMIT YOUR REQUEST, THE BETTER.
